Waiver

I hereby give my permission and approval for my child to participate in this activity. I assume all risks and such hazards incidental to such participation, and I hereby release The Forge Christian Ministries, property owner(s) and all the Volunteer Coaches from any and all liability arising from injury or injuries sustained by my child while participating in this event. The Representees assume no responsibility for any damage to or loss of any personal property. I hereby authorize the directors and employees of these agencies to obtain medical care for injuries and illness that might affect my child, or which might occur during these events. I further direct all medical or hospital facilities to accept this document as authorization to render emergency care to my child should it be deemed medically necessary.

Photo and Media Release

By registering my child for this event, I hereby grant The Forge Christian Ministries (The Forge) permission to photograph, record, or otherwise capture my child’s image, likeness, or voice during program activities. I understand that these photos or recordings may be used solely for promotional, educational, or marketing purposes directly related to The Forge (including print materials, website, and social media).

I acknowledge that my child’s name or personal information will never be shared outside The Forge. I understand that no compensation will be provided for the use of these images or recordings, and I waive any right to inspect or approve the finished materials.

This consent applies only to lawful and appropriate uses that represent the mission and activities of The Forge Christian Ministires and does not authorize use by any outside organization or for any unrelated purpose.
